Father’s Day Questionnaires

Did you have your students fill out their Mother’s Day Questionnaires for their moms, grandmas, and aunts this year?  Many of you asked if we could make Father’s Day Questionnaires too. In this download, there are 2 different versions of the questionnaire for dads, stepdads, grandpas, and uncles for your students who live with other loved ones.

This activity goes well with these resources:

Supplies:

  • Card Stock or Printer Paper
  • Printer
  • Father’s Day Questionnaires (click on the button below to print)
  • Pencils
  • Crayons or Markers
  • Thermal Laminator
  • Laminating Pouches or Self-Adhesive Laminating Sheets
  • Picture Frames

Set-Up:

  • Print one of the Father’s Day Questionnaires on card stock or printer paper for each student.

There are many different versions including Me and My Dad, Me and My Stepdad, Me and My Grandpa, Me and My Uncle, My Dad and Me, My Stepdad and Me, My Grandpa and Me, and My Uncle and Me.

Directions:

Before filling out our questionnaires, we always read books about dads, grandpas, and uncles. Yes… there are books about uncles! We like What Uncles Do Best by Laura Numeroff (What Aunts Do Best is on the other side of the book) and They Don’t Make Books About Uncles by Joe Jurek.

Our favorite books about dads is Just Me and My Dad by Mercer Mayer. For our questionnaires, we made “Me and My Dad” based off the books, but we also made the grammatically correct “My Dad and Me” version too.

After reading books about the important men in our lives, your students will write out their answers to the questions. If a child is having a difficult time writing, I usually write out answers on a white board for him or her to copy of find a buddy who can help.

Next, your students will draw a picture of their dad, stepdad, grandpa, or uncle.

Then, laminate the questionnaire or place it in a picture frame so it remains safe for many years to come.

Finally, you can have your students create their own wrapping paper for their Father’s Day presents. I always have my students dip cookie cutters into paint and stamp them onto large sheets of paper. Most of the parents love the wrapping paper just as much as the present!
